QuestionADHD Medication Concerta 18 + 36 + 54
• Initial Phases (18 & 36 mg): Had a good start, but efficacy dropped over time. Led to overthinking, distraction, low motivation, and heightened sensory sensitivity (light/sound).
• The 54 mg Dose: Provided zero benefit. Caused severe anxiety, intense pathological hyperfocus, deep isolation, and complete appetite suppression (causing weight loss). Discontinued after two weeks.
• Current Status (Back on 36 mg): Helps me get up and execute tasks, but focus remains very low. I experience extreme fluctuations, constant daydreaming, and poor task-switching.
• Social & Emotional Impact: Socializing triggers severe distraction and mind-wandering for 3–4 days post-event, especially if preceded by stress. My impulsivity has gradually worsened, and I feel constantly frustrated.
• Persistent Side Effects: Urinary retention, intense light/sound sensitivity, and body temperature fluctuations (severe morning chills followed by sudden heat).
Initially, I had total control over my focus. Now, it feels like a constant mental battle.

I had urinary retention issues too. During the first few days after increasing my Concerta dose from 36 to 54, I would pee but still feel like I had to pee more, and if I forced myself, I could keep peeing in little bursts for minutes. Thankfully, it went away after that.
I feel my skin has become more sensitive to the sun and summer heat. I hate the summer sun while taking Concerta and avoid it completely. As for the lighting at home or work, I think that is somewhat normal.
54mg sweat more, dry mouth sometimes, urge to smoke more.
Yes, I've been on Concerta for little over 6 months, and feeling good overall, but feel like not enough focus, tried 54 mg and I felt tired and confused, 54 feels way too much. I asked my doctor to send me for 36 again and feeling better but not enough.
