answered ADHD diagnosis report - help

Mine came from a psychologist after a full assessment, so it's probably way more detailed than what your doctor needs. Here's how it was laid out though, in case it helps you picture a report. The testing itself took about an hour, and I got the written report about a week later. Up top it has my name, birth date, age, the report date, and who referred me, plus a short section on why I was referred (to check my attention and thinking and figure out how to help). Then it lists the tests: an IQ test, the Conners Adult ADHD Scale, a continuous attention test, a memory test, and the Beck scales for depression, anxiety, and hopelessness. Every test gets its own section with scores, percentiles, and a range like average or below average, and it explains in plain terms what that means in daily life. The attention test covered impulse control and focus. For Conners it went through each subscale. It wraps up with a short summary (it says a psychiatrist has to confirm the final diagnosis), a small three-column table of main things to work on, smaller things, and strengths, and a few recommendations. Mine were extra time on tasks, breaks during long ones, and my psychiatrist reviewing the profile for medication.
